Insurance and Billing
Authorizations
Note that new conditions often require authorization before you can be treated. Please check with your insurance plan to see if you need one or if you can do a self-referral for hand therapy. In the state of California, self-referrals for physical therapy have been authorized. However, depending on your insurance plan, you should doublecheck that they will cover self-referrals for hand therapy before you make an appointment.
If your insurance plan requires a referral, you may need to see your primary care physician first before scheduling an appointment with one of our hand therapists.
Payments
Please remember that all co-pays are due at the time of your visit. For your convenience, we accept Visa and MasterCard. FORM Hand Therapy also offers cash and payment plans for qualifying patients. Our billing office is happy to answer your payment questions at (510) 505-1090.
Insurance Plans
Below is a list of the insurance that we currently accept at FORM Hand Therapy. We accept all PPOs and most EPOs, auto insurance, Medicare, and Workers’ Compensation. We will gladly accept most Palo Alto Medical Foundation plans. FORM Hand Therapy is not currently contracted with HMOs. If you do not see your insurance listed below, please call our billing specialists at (510) 505-1090 and they can answer any insurance-related questions you may have.
